Apply for a Grant

Do you need help getting a second opinion?

The Mike Shane Memorial Fund provides grants for patients to receive a free remote written second opinion, meaning you do not have to travel. A second opinion can provide an alternative diagnosis or treatment plan or even confirm your initial diagnosis.

To qualify for this grant and receive this service, the patient must meet the following criteria:

  1. Has received a diagnosis of cholangiocarcinoma

  2. Not currently - nor will be at the time of consultation - a registered inpatient at any hospital

  3. U.S. Citizen or lawful Permanent Resident

If the patient's answer is yes to all of the above questions, please fill out the form on the following page to begin the application process. Simply click the button"begin application' to continue. Note that you do not need to provide insurance information. 

 

What to Expect:

  • Upon submission, a representative from the Mike Shane Memorial Fund or our partner, Purview, will be in touch via phone or email regarding the next steps. This usually takes 1-2 business days.

  • Once eligibility is confirmed, you will be awarded a grant from the Mike Shane Memorial Fund for receiving a free online second opinion (all from the comfort of your home - no traveling involved!).

  • You will have the ability to choose from one of our partnering institutions where you would like to receive your remote second opinion.

  • Our partnering institutions will confirm their ability to review your case, help you to collect your medical records, document questions, any follow-up, and share the second opinion report back with you generated from an expert physician or physician team.

  • Online second opinions usually take 2-3 weeks, depending on how quickly medical records can be collected and a few other factors.

  • Once the second opinion is complete, The Mike Shane Memorial Fund is billed by the partnering institution, and pays directly on the patient's/ grantee's behalf.

We will not request any medical information or details about your second opinion, only feedback regarding the application process to help spread the word for future grant recipients.


The Mike Shane Memorial Fund and all medical partners, have the right and ability to deny or refuse any request for a second opinion.
